老化增加衰弱問題發生的機會,老年人的生活功能、健康自我管理對預防及延緩老年人衰弱問題,和 維持健康生活品質具有重要性。 本研究目的是探討老年人的健康狀況、健康自我管理能力與健康生活自我效能對社區老年人衰弱與健康生活品質的影響,並驗證預防衰弱健康自我管理模式,以協助社區老年人預防衰弱及提升生活品質。 本研究採橫斷面量性研究,分為三階段進行。第一階段研究目的以Tilburg Frailty Indicator (TFI) 測試及建立多元衰弱評估之台灣版工具 (TFI-Taiwanese version; TFI-T),並確定台灣老年人的評斷衰弱的截斷點分數。第二階段研究目的分析衰弱

對生活品質之影響,及分析日常生活活動能力及健康生活自我效能中介衰弱對生活品質之影響。第三階段研究目的為以健康促進與自我管理概念建立預防衰弱健康自我管理模式,並以結構方程式進行模式驗證。本研究共招募210位居住於台灣中部之社區老年人(M=74.45歲, SD=9.15),女性占75.5%,能執行每日之日常生活活動,並參加社區關懷據點的活動。參與的老年人進行個別訪填寫問卷:人口學變項、慢性疾病與健康問題量表、健康功能量表、健康自我管理量表、健康生活自我效能量表、衰弱檢測工具及生活品質量表。 第一階段研究結果顯示TFI-T與TFI的生理、心理及社會衰弱結構相同,具有良好的適配度,並具有良好的信

度與效度。ROC曲線分析表明TFI-T衰弱之截斷點大於5.5分可認定為衰弱,此截斷點具有敏感度與準確性。結果顯示本研究所建立之TFI-T為具信度與效度之多元衰弱評估工具。 第二階段以Hayes之Model 6進行序列中介效果分析,研究結果顯示日常生活活動能力及健康生活自我效能,具有二變項序列多重中介作用,可部分中介降低衰弱對生活品質之負面影響。 第三階段以Amos 24.0進行結構方程模式統計分析,再進行整體模式修正後,資料顯示「預防衰弱健康自我管理模式」之結構方程模型適配度指標良好,代表老年人資料與本研究建構之模型間具契合度。有關「預防衰弱健康自我管理模式」中各變項之路徑分析結果

顯示如下: 1.「健康生活自我效能」對「生活品質」具有顯著之直接預測力;2. 「健康生活自我效能」 可顯著部分中介「衰弱」、「日常生活活動能力」對「生活品質」之解釋力。3.「衰弱健康自我管理」、「 運動自我管理行為」,可以顯著預測「健康生活自我效能」。4.「衰弱」經「日常生活活動能力」、「衰弱健康自我管理」、「健康生活自我效能」等變項之多重中介,顯著減弱對老年人「生活品質」之影響。在本模式中「衰弱健康自我管理」之認知態度與行為影響「健康生活自我效能」,同時,「健康生活自我效能」是影響衰弱與生活品質關係之最重要的中介因素。 由本研究建構之模式可作為評估社區老年人衰弱狀況,發展預防衰弱之方案
