背景:以1995年於北部發生的院內瘧疾感染事件為契機來探討醫院員工對病人安全文化的認知是否會影響其組織公民行為的表現。目標:探討中部某公立醫院員工對病人安全文化與組織公民行為的相關性。方法:本研究對個案醫院全部員工採取橫斷式問卷調查,量表分三部分,第一部分為人口學變項,包含:性別、年齡等15題項。第二部分為「病人安全文化」,並取得財團法人醫院評鑑暨醫療品質策進會授權使用,共計30題。第三部分為「組織公民行為」,參考國內外研究者發表之期刊論文設計15題,以李克氏特5點衡量。經專家效度檢定CVI值為0.91,Cronbach's α值為0.952。有效回收問卷共603份,有效回收率為75.38%

。資料以SPSS 25.0統計軟體進行分析。結果:結果得知病人安全態度量表的平均數為3.89。在六個構面中,以「團隊合作風氣」構面的平均數4.01為最高,「對壓力的認知」構面的平均數3.66為最低。組織公民行為量表的平均數為4.24。在五個構面中以「禮貌」構面及「責任心」構面平均數最高皆為4.48,「體育精神」構面平均數3.93則為最低。差異性分析結果得知不同性別、是否擔任主管、有無宗教信仰、教育程度、院內年資及僱用類別在病人安全文化及組織公民行為之構面有統計學顯著差異。病人安全文化中有五個構面包含「團隊合作風氣」(r= 0.65)、「安全的風氣」(r= 0.65)、「對工作的滿意度」(r=

0.69)、「對管理部門的感受」(r= 0.60)、「對工作環境的感受」(r= 0.66) 與組織公民行為呈顯著正相關。「對壓力的認知」和病人安全文化中的「對工作的滿意度」構面 (r= -0.04)、「對工作環境的感受」構面 (r= -0.03) 及組織公民行為中的「公民美德 」構面 (r= -0.02)、「體育精神」構面 (r= -0.05) 則呈現顯著負相關。線性迴歸結果得知病人安全文化中,「對工作的滿意度」及「對工作環境的感受」與組織公民行為及其五構面皆有關聯性。結論:樣本醫院員工除「對壓力的認知」外,病人安全文化及其中五構面與組織公民行為及其五構面有正相關性。管理者可藉由提升員工對工作

台灣飲食綱領為基礎的健康飲食與老人衰弱症及內在潛能之相關與介入研究

前言:健康老化係指發展和維持老年生活福祉所需功能性能力的過程;其中以提升身體和心理的內在潛能為健康老化的核心。飲食和營養是健康老化最重要的基石。研究發現高抗氧化、抗發炎營養密度的飲食型態,如:得舒飲食和地中海飲食與降低衰弱症風險和提升內在潛能有關,唯台灣飲食和西方飲食仍異。本研究目的為探討台灣飲食綱領依從性與健康老化之關係,並探討飲食營養作為增進內在潛能之策略。材料方法:本研究154名安養機構和社區受試者資料取自於一科技部整合型計畫。研究採橫斷型與介入型兩階段研究設計。以橫斷式研究設計,探討154名安養機構和社區受試者得舒飲食分數(DASH)、地中海飲食分數(MDS)與本研究發展的台灣健康飲

食評量(T-HEI)分數等飲食品質與衰弱症之關係。以準實驗研究設計,針對59位安養機構住民進行四個月高抗氧化抗發炎營養密度飲食介入研究;並與機構營養師溝通供餐菜單調整以符合每日飲食指南之建議,輔以營養衛教提住民研究遵從度。於介入研究第二及第四個月分別測量住民T-HEI分數、內在潛能和功能性能力變化情形。結果:校正可能的干擾因子後,T-HEI顯著預測衰弱症分數 (β= -0.15 ± 0.01, p= 0.033);高抗氧化抗發炎營養密度食物如:全穀、深色和橘黃色蔬菜、堅果種子攝取隨衰弱之嚴重有顯著減少的趨勢。控制介入前差異和可能干擾因子後發現四個月飲食介入遵從度高的老人心肺耐力(Pintera

ction= 0.009)和步行速度(Pclusters= 0.034)顯著提升;下肢肌力和認知功能則隨時間顯著下降。內在潛能的提升進一步改善日常生活功能(ADL)的表現 (Pinteraction= 0.034)。結論:以飲食綱領為基礎、富含抗氧化抗發炎營養密度食物的健康飲食證實能顯著促進健康老化。高抗氧化抗發炎營養密度食物攝取不僅提升台灣老年人飲食品質亦顯著增強其內在潛能和功能性能力。
