中文摘要研究背景尿路結石患者經急診治療返家後,再度來診情況頻繁,除病患不便並易造成醫療資源耗費,顯見現行尿路結石治療指南仍有不足,故需探討再度來診之相關危險因子。研究目的探討尿路結石患者急診治療後再度來診相關影響因子。研究方法本研究採用回溯性世代追蹤研究設計(retrospective cohort study),回溯三總醫院急診部105年1月1號至109年12月31號,所有診斷尿路結石共9383筆資料,排除無腹部電腦斷層檢查共收納1575人,其中184人為七十二小時內再度來診病患。描述性統計用t test 以及 卡方檢定分析有無再度來診之基本人口學分佈及生化檢驗等數據,推論性統計用羅吉斯回
歸預測尿路結石再度來診之危險因子。研究結果整體上,於急診室有接受非類固醇藥物治療風險值為1.53倍,(95%CI=1.10-2.12)。生化檢驗部分,慢性腎疾病第三期b風險值2.85倍,(95%CI=1.53-5.32)。尿路結石部位,輸尿管結石相較於腎結石,再度來診風險為6.90倍(95%=2.51-18.99);遠端輸尿管結石風險值為1.44倍(95%CI =1.00-2.07),左側輸尿管結石風險為1.40倍(95%CI=1.00-1.95),雙側輸尿管結石為4.31倍(95%=1.56-11.94)。尿路結石體積部分,輸尿管結石體積增加0.1公分,再度來診風險為1.61倍(95%CI=
