參與式之讚賞行動與反思介入對於長期照顧機構照顧服務員之工作滿足感與離職意圖之成效

背景:隨著國內失能人口數的增加,照顧服務員的需求相對增加。但照顧服務員人力供需失衡,需求遠大於供給。住宿式長照機構的照顧服務員存在著招募不易及離職率高的潛在問題,影響照顧品質甚鉅。目的:本研究目的主要在於探討參與式之讚賞行動與反思(Participatory and Appreciative Action and Reflection; PAAR)介入措施後,長期照護機構照顧服務員在工作滿足感與離職意圖的改變情形。方法與材料:採類實驗型研究設計,立意取樣2家北區退輔會體系下之榮民之家,提供照護工作之委外照服員共112名。依榮家主管意願將其中1家分配為介入組56名,另1家為對照組56名。兩組

按常規給予在職教育訓練,但介入組自2019年5月起額外接受為期三個月的PAAR介入措施。使用工作滿足感及離職意圖量表,分別於PAAR介入前、介入後一週、介入後一個月進行三次測量,以廣義估計方程式(Generalized Estimating Equation, GEE)檢定介入成效。另以內容分析法(content analysis method)分析PAAR介入過程中18場次的團體介入文本。結果:全程參與本研究之照顧服務員共計92人,其中介入組47人;對照組45人。參與對象以女性居多(79.3%);多為年齡50歲以上中高齡者(68.5%);教育程度以國中、高中學歷者,各佔約3成。PAAR介入過

程發現,介入組在過程中能夠藉著團體互動,正向看待工作,並且從中找到工作的成就感。此外,團體中的相互支持,也能幫助個人修正工作的方式,達到同儕互助並且相互讚賞。透過統計檢定發現,PAAR介入措施一個月後,介入組工作滿足感顯著高於對照組(β=5.08;p
